Tiny fighter: SUM Hospital doctors save 1.47-kg premature baby girl after complex lung surgery

Bhubaneswar, Nov 5 (UNI) Doctors at SUM Hospital, Bhubaneswar, have scripted a remarkable medical success by saving a premature baby girl weighing just 1,476 grams at birth.
Born with a rare congenital lung disorder, the newborn underwent a complex lung surgery and overcame multiple life-threatening complications following weeks of intensive medical care and dedicated teamwork.
The baby, delivered at 34 completed weeks of gestation through an emergency Lower Segment Caesarean Section (LSCS), was born to a 28-year-old first-time mother following an in-vitro fertilisation (IVF) conception.
Soon after birth, the newborn developed severe respiratory complications and required immediate intubation and mechanical ventilation.
The medical challenges had been identified even before delivery. An antenatal scan conducted at 20 weeks of gestation had detected a mixed cystic Congenital Pulmonary Airway Malformation (CPAM), an abnormal growth of lung tissue, along with an echogenic lesion in the left lung and a dextroposed heart.
Following admission to the hospital, the baby was initially placed on mechanical ventilation. As her condition remained critical, the medical team upgraded her respiratory support to High-Frequency Oscillation (HFO) ventilation.
A Contrast-Enhanced Computed Tomography (CECT) scan of the chest subsequently confirmed left-sided Congenital Pulmonary Airway Malformation, accompanied by right-sided pneumothorax.
Further evaluation by the paediatric cardiology team, supported by a 2D echocardiogram, also revealed pulmonary hypertension, adding to the complexity of the newborn's condition.
Considering the severity of the congenital lung abnormality, the paediatric surgery team decided to undertake definitive surgical intervention.
On the 15th day of life, the baby underwent a Left Pneumonectomy, a major surgical procedure involving the removal of the affected left lung. Both the upper and lower lobes of the left lung were successfully excised.
The post-operative period presented further challenges, as the baby required treatment for pulmonary hypertension, infection and coagulopathy. She also needed multiple blood component transfusions, intensive fluid management and parenteral nutrition, through which essential nutrients were delivered directly into the bloodstream.
With carefully monitored ventilatory support and coordinated treatment by the multidisciplinary medical team, the baby's condition gradually improved. Her respiratory status showed steady progress, allowing doctors to progressively reduce ventilatory assistance.
A major milestone was achieved on the 24th day of life when the baby was successfully extubated. She continued to recover and was eventually weaned off respiratory support, maintaining normal oxygen saturation in room air by the 50th day of life.
The newborn was discharged from the hospital a week later after demonstrating satisfactory weight gain and the ability to tolerate breastfeeding as well as bottle-feeding with expressed breast milk. At the time of discharge, she was neurologically normal, maintaining oxygen saturation without supplementary oxygen and showing no signs of respiratory distress.
The complex surgical intervention was performed by a team led by Dr. Antaryami Pradhan of the Department of Paediatric Surgery.
Expressing immense pleasure over the successful outcome, Head of the Department of Neonatology Dr. Debasish Nanda congratulated the entire team for its dedication, commitment and exemplary teamwork in managing the extremely challenging case.
