KIMS Kondapur surgeons successfully remove massive retrosternal goiter in high-risk 7-hour surgery

Hyderabad, May 17 (UNI)In an exceptional display of surgical expertise and multidisciplinary coordination, doctors at KIMS Hospital, Kondapur, successfully removed a rare and massive retrosternal goiter from a 35-year-old woman in a high-risk surgery lasting over seven hours.
In a statement here on Saturday, the case details were shared by Consultant Thoracic Surgeon Dr Rohan Reddy C.
The patient initially presented to a local hospital with moderate chest pain and occasional breathing difficulty. Imaging revealed a large mass occupying a significant portion of the right upper chest cavity. Following a biopsy, which suggested the mass resembled thyroid tissue, she was referred to KIMS Kondapur for further evaluation.
At KIMS, advanced diagnostics confirmed a long-standing thyroid swelling that had extended from the neck deep into the chest cavity, compressing the right lung, heart, and major blood vessels—classic features of a massive retrosternal goiter. Clinical examination also revealed swelling of the right arm and prominent chest veins, indicating Superior Vena Cava (SVC) Syndrome—a potentially life-threatening condition caused by venous congestion.
After a comprehensive multidisciplinary discussion, the patient and her family were thoroughly counselled about the complexity and high risks associated with the surgery. The surgical team undertook a demanding procedure involving a sternotomy (vertical splitting of the breastbone) and a right anterior thoracotomy. This "clam-shell" approach—lifting the chest wall—allowed complete access to the mass.
The tumour, measuring over 15 cm, was found to be compressing the upper right lung, heart, and right brachiocephalic vein. Thanks to the precision and perseverance of the surgical team, the entire mass was successfully excised, restoring normal blood flow and alleviating compressive symptoms.
Postoperatively, the patient made an impressive recovery. With continuous monitoring and rehabilitation support, she was safely discharged within a week.
The multidisciplinary surgical team included Thoracic Surgeon Dr Rohan Reddy C, Endocrine Surgeon Dr Ramya Vailveru, Cardiac Surgeons Dr. Nisarga and Dr. Vineet, Vascular Surgeon Dr Venkatesh Bollineni, and the Anesthesia team.
“A massive retrosternal goiter is a rare and dangerous condition that often goes unnoticed until serious symptoms develop,” said Dr Rohan Reddy. “Complete excision is the definitive treatment, and we are proud that our team has both the expertise and infrastructure to perform such a complex surgery successfully.
